Supreme Ceiling Fan Model E&G Type. Made in Taiwan, sold and distributed by Supreme Ceiling Fans. This ceiling fan has a three speed pull chain, forward/reverse switch. This ceiling fan is from the early to mid 1980s, 1983 - 86 era. Not exactly sure if this fan is a Supreme, but it's defiantly related to those types of ceiling fans.
